We are currently in Phase 1a and initial groups from Phase 1b.
Eligible New Yorkers in Phase 1a and 1b are:
- High-risk hospital workers (emergency room workers, ICU staff and Pulmonary Department staff)
- Residents and staff at nursing homes and other congregate care facilities
- Federally Qualified Health Center employees
- EMS workers
- Coroners, medical examiners and certain funeral workers
- Staff and residents at OPWDD, OMH and OASAS facilities
- Urgent Care providers
- Individuals administering COVID-19 vaccines, including local health department staff
- All Outpatient/Ambulatory front-line, high-risk health care workers of any age who provide direct in-person patient care
- All staff who are in direct contact with patients (i.e., intake staff)
- All front-line, high-risk public health workers who have direct contact with patients, including those conducting COVID-19 tests, handling COVID-19 specimens and COVID-19 vaccinations
- This includes, but is not limited to,
- Doctors who work in private medical practices and their staff
- Doctors who work in hospital-affiliated medical practices and their staff
- Doctors who work in public health clinics and their staff
- Registered Nurses
- Specialty medical practices of all types
- Dentists and Orthodontists and their staff
- Psychiatrists and Psychologists and their staff
- Physical Therapists and their staff
- Optometrists and their staff
- Pharmacists and Pharmacy Aides
- Home care workers
- Hospice workers
- Staff of nursing homes/skilled nursing facilities who did not receive COVID vaccination through the Pharmacy Partnership for Long-Term Care Program

- Public Safety Communications
- Emergency Communication and Public Safety Answering Point Personnel, including dispatchers and technicians
- Other Sworn and Civilian Personnel
- Court Officers
- Other Police or Peace Officers
- Support or Civilian Staff for Any of the above services, agencies, or facilities
- Fire Service
- State Department of Corrections and Community Supervision Personnel, including correction and parole officers
- Local Correctional Facilities, including correction officers

- Licensed, registered, approved or legally exempt group Childcare Providers
- Employees or Support Staff of Licensed or Registered Childcare Setting
- Grocery store workers
- Public Transit
- Airline and airport employees
- Passenger railroad employees
- Subway and mass transit employees (i.e., MTA, LIRR, Metro North, NYC Transit, Upstate transit)
- Ferry employees
- Port Authority employees
- Public bus employees
- Individuals living in a homeless shelter where sleeping, bathing or eating accommodations must be shared with individuals and families who are not part of the same household
- Individual working (paid or unpaid) in a homeless shelter where sleeping, bathing or eating accommodations must be shared by individuals and families who are not part of the same household, in a position where there is potential for interaction with shelter residents
Step 1: Determine eligibility and schedule an appointment. Vaccines are available at pharmacies, hospitals and through local health departments statewide – please contact your provider of choice to schedule your vaccine appointment.
Additionally, there is a network of State-run sites distributing vaccine statewide – to determine eligibility and schedule appointments at New York State-run vaccination sites only, use the Am I Eligible app. You can also call the New York State COVID-19 Vaccination Hotline at 1-833-NYS-4-VAX (1-833-697-4829).
Due to extremely limited supply from the federal government, it may be 14 or more weeks before an appointment is available.
Step 2: Complete the Vaccine Form. Once you have a confirmed appointment, you must complete the New York State COVID-19 Vaccine Form. This form can be completed online and you will receive a submission ID, or can be completed at your vaccination site. (We encourage New Yorkers to complete the form ahead of time.)
Step 3: Bring proof of eligibility to your appointment. When you go to your vaccination site for your appointment, you must bring proof of eligibility. This may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status. If you are eligible due to age, you should bring a form of ID (like a Driver's License or passport) that includes your date of birth. You will need to complete a clinical questionnaire form at the time of your appointment.
Step 4: Schedule your second appointment in-person at your vaccination site. You must schedule an appointment to get your second shot at your vaccination site, in-person, immediately after getting your first shot. Remember: Both vaccines require two doses for effectiveness.
Instructions for New York State-Operated Vaccination Sites:
Step 1: Determine eligibility and schedule an appointment. The Am I Eligible app is the quickest way to see if you're eligible and make an appointment. You can also call the New York State COVID-19 Vaccination Hotline at 1-833-NYS-4-VAX (1-833-697-4829). Once you have successfully scheduled an appointment, you will receive a confirmation email that contains a barcode. You will need to bring this to your appointment.
Step 2: Complete the Vaccine Form. Once you have a confirmed appointment, you must complete the New York State COVID-19 Vaccine Form. This form should be filled out online and you will receive a submission ID indicating completion. You will need to bring the submission ID to your appointment. If you cannot submit the form online, it will be available at the vaccination sites.
Step 3: Bring proof of eligibility to your appointment. Depending on your eligibility category, proof can include an employee ID card, a letter from an employer or affiliated organization, a pay stub, a driver's license, passport, or any legal proof of your date of birth and residency. See full list below. At the time of your appointment, you'll be asked a series of clinical questions to ensure readiness for a vaccine. You will be asked for insurance information BUT the vaccine is free and there will never be a charge to you. This information is for administrative use only.
Step 4: Your second dose appointment will be scheduled automatically when you receive your first vaccine dose. Your second appointment will be scheduled for the same time and at the same location, three weeks following your first dose. You will receive a card onsite with the date and time indicated and a confirmation email will follow a few days later. Please keep in mind when scheduling your first appointment that your second appointment will occur at the same time of day.
Acceptable forms of proof of eligibility
If an individual is eligible due to their employment status, they must prove they are employed in the State of New York. Such proof may include:
• an employee ID card or badge,
• a letter from an employer or affiliated organization, or
• a pay stub, depending on the specific priority status.
If an individual is eligible due to their age, they must produce proof of age and proof of residence in New York. To prove New York residence, an individual must show:
• One of the following: State or government-issued ID; Statement from landlord; Current rent receipt or lease; Mortgage records; or
• Two of the following: Statement from another person; Current mail; School records.
For age, such proof may include:
• Driver’s license or non-driver ID;
• Birth certificate issued by a state or local government;
• Current U.S passport or valid foreign passport;
• Permanent resident card;
• Certificate of Naturalization or Citizenship;
• Life insurance policy with birthdate; or
• Marriage certificate with birthdate.
New York State based its COVID-19 vaccine distribution and administration process on ten guiding principles.
- Safety: New York State will only endorse and distribute a COVID-19 vaccine if it is determined to be safe and will only be used according to the indication under which it received its authorization or license. This includes continued monitoring and reporting of adverse events after the vaccine is licensed and administered.
- Effectiveness: New York State will only endorse and distribute a COVID-19 vaccine if it is demonstrated to be appropriately effective in the populations intended for use.
- Expert approved: New York State will rely on the advice and counsel of recognized clinical experts and scientists to review and approve the safety and effectiveness of every vaccine that is authorized by the federal government for distribution.
- Equitable & clinically driven distribution: New York State’s COVID-19 vaccine distribution plan will be based on standards that prioritize people at higher risk of exposure, illness and/or poor outcome. Unrelated factors, such as wealth or status, will not influence distribution.
- Transparency: Throughout the COVID-19 crisis, the state’s daily public presentation of facts and reliance on science and medical expertise helped build public trust and confidence in government action. New York State will continue to be transparent regarding all aspects of the COVID-19 vaccine distribution, administration, and monitoring process to ensure New Yorkers are fully informed.
- Use of Data: Coordination of a successful vaccination program will require robust tracking, data and analytics capabilities. New York State will use powerful data and information technology platforms to guide all parts of the COVID-19 vaccine distribution and administration process to maximize safety, accuracy, and efficiency and meet all federal reporting requirements — all while maintaining patient privacy.
- Privacy and Patient Safety: New York State will ensure all vaccination processes prioritize patient safety, and all information systems guarantee patient privacy. Vaccination does not take away the importance of other public health measures that have served us well in the fight against COVID-19. New Yorkers will be urged to continue to practice social distancing, mask wearing, hand washing, and other measures.
- Partnership, Coordination & Public Outreach: New York State recognizes that coordination with local organizations and community providers is essential to the safe and successful distribution and administration of COVID-19 vaccines. The state’s outreach efforts will especially focus on reaching underserved, hard to reach, and vaccine-hesitant populations, as well as those at highest risk for COVID-19 infection and poor outcomes.
- State Leadership: New York State expects federal engagement on vaccine vetting, distribution, administration, and funding. However, regardless of the level of federal engagement, New York State will take all necessary steps and require local coordination with the state’s centralized approach to ensure an efficient and organized vaccine distribution.
- NEW YORK TOUGH: Throughout this COVID-19 crisis, New Yorkers have shown that there is nothing we cannot do if we work together as one community. Our approach to the COVID-19 vaccine will be tough, strong, united, disciplined, and loving.
Pharmacy Partnership for Long-Term Care Program
New York has opted into the federal government's Pharmacy Partnership for Long-Term Care Program for COVID-19 vaccination. Under the federal program, employees of CVS, Walgreens and other select pharmacies will vaccinate residents and staff in long-term care facilities including nursing homes, much like the do for the flu vaccine.
The program began on December 21, 2020. New York has dedicated doses from our initial allotments to this program and will dedicate future allotments to help complete the program and ensure all residents and staff at long-term care facilities are vaccinated. There are approximately 600 nursing homes across the state that are participating in the program.
National Guard Vaccination Pilot Program
New York National Guard has been selected by the Department of Defense as one of 16 pilot programs across the nation to be part of the limited distribution of the Pfizer COVID-19 vaccine to military personnel. Members of the New York Army and Air National Guard who serve as part of the state's COVID response efforts will be eligible for the vaccine.
